Consent Form

Purpose of the Study: This study is being conducted to help us learn more about people who are seeking help or feedback on their own or the drinking of a loved one. You are being asked to participate because you meet the requirements to participate in this study.

Procedures: If you agree to participate, you will be asked to fill out questionnaires about yourself such as your or another’s drinking history, motivation to change, barriers to change, intervention preferences, and what you think is the best way to help people change their drinking. The questions will take about 15-20 minutes to complete. This is a one time questionnaire.

Risks: You may feel uncomfortable answering certain questions about yours or another’s drinking. You can skip any questions or stop the questionnaire at any time.  We attempt to keep your information protected but cannot guarantee the privacy of your answers. We specifically do NOT collect any identifying information, keep your IP address separate from your data our database and our site uses a secure socket layer, which encrypts all information as it is transmitted. This will help ensure that you are not identified by outside parties.

Benefits: The potential benefits of this study are that you are helping us develop more individualized online interventions for those seeking help or feedback on their own or another’s drinking.

Alternatives: You are free to choose not to be in this study. Stopping or not participating in this survey will NOT affect your ability to use all the services of AlcoholScreening.org such as the screening, feedback, treatment resources or intervention.

Costs: There are no costs to participating in this study.

Payment: You will not be paid for participating in this study.
